Here are some forms you may use when requesting. If you use the 'other' form, please post here and notify what you want, and I may add a form for that to this list.
You don't have to stick to the format of the form, though that might be easiest, but you must include everything.
Mate:
Name: Your warrior cat name.
Gender: She-cat or Tom?
Age: In moons.
Clan: Which clan?
Link to Bio: A link. You must have made a bio to request a mate.
Approximate Activity: How often are you on, approximately? DO NOT LIE, YOU WILL BE PUNISHED. Admin//staff reads these, and if you are not on as much as you said you are, you will receive a consequence.
Already have kits? Do you already have kits that THE MATE YOU'RE REQUESTING is the mother/father of? Give links to bios and names if so.
Previous mates? Kits? Do you have any previous mates? Link to bio (if any) and name.
Number of Kits Wanted: How many (even if it's approximate, or 'I don't care') you want.
RPers for Kits: Do you have any people to RP your kits yet? If so, who?
Audition?: Do you want people to audition for the role of your mate?
Scenario: If so, what's your scenario? (Ex. I'm feeling lonely, and you walk up. What do you do/say?)
Other: Anything else?

Any plots before death involving killer? If so, what? Does the killer need to do anything before he/she actually kills you? Such as make an enemy of you, insult you, etc.
What should/can killer do after death? Anything they want to cover up evidence, or what else? Do you want them to be sure to be caught, or to get away with it? Or is it all up to them?
Audition? Do you require an audition?
Scenario: If so, what's your scenario? (Ex. Examplepelt padded through the ExampleClan camp entrance, heading off to hunt. She/he stalked through the undergrowth, eyes trained on a mouse, but little did Examplepelt know, another pair of eyes were trained on him/her! What are you doing/thinking while watching me and plotting?)
