Explanations:

Name: Your cat name here.
RP Thread Title: The exact title of the thread.
Archive Board: Which board is the archive in? ShadowClan? OOC? etc.
Page #: The page number of the archives in which it can be found (We don't want to search through a ton of pages because your RP is really old.)
Reason: A continuation of the RP, or because we made a mistake?
RP Idea(s): A very general drift of where you're going to take this RP, if it is a continuation.
