Explanations:

Contest Name/Thread Title: The name of your contest, which should also be the title of your contest thread.
End Date: The ending date of your contest, doesn't have to be exact. 'Last week' is fine.
Your Username: The name you use to login.
Winner1 Display Name: The display name of the first winner.
Winner1 Username: The name the first winner uses to login. (You can ask, or, if you click on 'members' in the menu bar, find the person, click on their name, and look in the address bar, the username is at the end.)
Winner1 Fresh-Kill Reward: How much Fresh-Kill, if any, they earned.
Winner1 Other Reward(s): Anything else they earned.
